***If you are applying as an orchestra member, you must send a cassette or CD audition with this application and a letter of recommendation from your conductor. If you are applying as a member of the chorus, a letter explaining your experience suffices. References are appreciated. Former enrollees need not send a letter of recommendation or audition materials.

Festival fees must be paid by check or money order and made payable to University of South Florida.

I have read the Festival information in the brochure or on the website, all inserts and appended materials and agree to comply with all regulations set forth therein. If I have any questions concerning subsequent information, I shall communicate those questions in writing as soon as possible. Included with this application is my deposit by  check or money order for US $100, which is a non-refundable administrative fee. All fees are due by June 1, 2008 and become non-refundable after that date except in cases of certifiable emergency explained in writing.

A deposit of $100 by check or money order made payable to University of South Florida must accompany this form. Please send your application and deposit to  University of South Florida, Attn: Classical Music Festival, 4202 E. Fowler Avenue, FAH 110, Tampa, FL 33620-7350. DO NOT SEND CASH! Applicants should expect to hear from us regarding their acceptance into the program within two weeks from the date we receive complete application materials and deposit. If your application is rejected by us, we will return your entire deposit. Thank you.
